Who we care for
We provide adults with CF in Victoria access to the best available medical, nursing and allied health care to preserve and enhance their best possible quality of life.
The members of the CF team who you see in the outpatient clinic will also be responsible for your management in hospital and work closely with the ward medical and nursing staff.
We can also provide treatment for you in your home, when you are well enough, using the Hospital in the Home service.
What to expect
The responsibility for your health management ultimately rests with the CF specialist consultant or physician.
However, many years of research and experience has shown us that a multi disciplinary approach to care is the most helpful for people with CF.
When you are in clinic, you will meet several health care professionals who have different, but complimentary skills and qualifications, all aimed at providing you with the best overall health care advice.
How to access this service
Transfer information or a referral from your GP
To be seen in one of our specialist outpatient clinics you need to be referred by your GP, another specialist doctor or your Alfred Health doctor.
Your referral will need to be emailed to op.referrals@alfred.org.au or faxed to (03) 9076 6938.
If you have a mobile number, SMS confirmation that we have received your referral as soon as we enter it into our system. This is usually within 24-48 hours. If you do not have a mobile number you will receive this notification via post.
Your referral will then be reviewed by a doctor or nurse to determine how urgently care needs to be provided.
Depending on the clinical urgency of your medical condition we will then contact you to advise of an appointment time or that you have been placed on a waiting list for an appointment
What to bring
For your appointment
- Your appointment letter. This can be on your phone or a hardcopy.
- Your Medicare or DVA card and any valid concession cards
- An up-to-date list of your medications, this can be an electronic list on your phone.
- Any relevant tests or investigation results that you have had done in preparation for your appointment
