National Partnered Medication Charting Model (PPMC) Credential

Alfred Health Pharmacy, in partnership with the Society of Hospital Pharmacists Australia (SHPA), has launched the National PPMC Credential program. PPMC is an innovative model of care which sees appropriately credentialed pharmacists work collaboratively with authorised prescribers to undertake a medication review and chart medications for nursing staff to administer. This model has been shown to reduce medication errors, length of hospital stay and hospital costs.
