Australian Disease Management Association
The Australian Disease Management Association (ADMA) is currently a clearing house of expertise in disease management.
It provides a forum and advocacy for disease management services to promote programs, which improve outcomes and quality of life, and reduce acute health care utilisation and costs in patients with chronic conditions.
ADMA is a not-for-profit group which represents all stakeholders in the industry and provides educational activities to health care organisations, providers, funders and health care policy makers on disease management.
There has been substantial growth in the awareness and implementation of disease management initiatives across Australia. There are various models or components to Disease Management which include self-management, case management, behaviour change, health risk assessments, telephone or online coaching, outreach visits, coordinated clinics, disease specific programs and programs for complex co-morbid patients. These occur in a range of settings including acute hospitals, general practice, community health, private health funds, call centres, workplaces and home environments.
Information exchange of disease management experience and evaluations continues to be integral in determining quality, effectiveness and credibility.
