For GPs

At Alfred Health, the GP Liaison team aims to improve communication between General Practitioners and Alfred Health to enhance patient care. 

We can provide support by:

  • Facilitating communication between Alfred Health and GPs in the community, including HMO education and improving discharge and outpatient correspondence
  • The development and implementation of strategies to assist in outpatient clinic demand management and facilitate patient care
  • Maintenance and updating of referral guidelines
  • Assisting GPs in accessing hospital services, and providing a point of contact for GPs
  • Liaising with the Primary Health Networks on a number of projects, including GP and practice nurse education programs
  • Encouraging patients to have a GP

COVID-19 impact - Specialist Clinics

The impact of COVID-19 has resulted in high demand for specialist consultations. If there is any concern about the delay of the appointment, or any deterioration in the patient's condition, please send an updated referral with additional information. If the patient's care needs have become urgent, please call the unit registrar on (03) 9076 2000.

If you would like Rich Text Format (.RTF) files of referral forms for integration with your practice software, please contact GP Liaison.

Our referral guidelines outline the triage process and provide information about:

  • When to refer
  • What information, including investigations, are required in referrals
  • The priority for each condition listed

Request clinical patient information

Requests for Alfred Health correspondence, such as outpatient letters and discharge summaries should be directed to Health Information Services.

GP emergency hotline

The Alfred Emergency and Trauma Centre has a dedicated 24-hour hotline for GPs who require urgent clinical advice from an Emergency Physician and decision support for urgent referrals to services at Alfred Health.

GPs can phone 1800 ALFRED (1800 253 733) for emergency clinical advice 24/7, and are encouraged to call prior to sending a patient to ED where possible.

A written referral is not required; however, GPs can send clinical information when directing a patient to ED by Fax (03) 9076 6063.

Statewide referral criteria for specialist clinics

The Victorian Department of Health and Human Services has developed statewide referral criteria to assist GPs and clinicians referring patients to specialist clinics. These referral criteria have been developed to improve access to specialist clinics in public hospitals by improving the quality and appropriateness of referrals.
