Clinical service overview
We offer specialist assessment and therapy from the following disciplines:
- Geriatric assessment
- Pain management
- Falls risk assessment
- Chronic illness management
- Medication review and management
- Complex wound care
- Catheter management
- Blood monitoring including warfarin
- SC/IM/IV medication administration
- Rehabilitation post orthopaedic surgery
- Physical therapy
- Home equipment assessment
- A ‘Welcome Home’ visit can be provided on the day of discharge from the ward if required
- A ‘Welcome Home’ meal is offered to those who it is indicated for.
We have the ability to broker services as needed, including carers who can assist with mobility, personal care, shopping, meals, domestic tasks and/or respite.
Please note, brokerage of services for Better at Home patients is determined on a case-by-case basis and is only accessible when there is no other available assistance through family/friends/carers.
GPs, please note, Better at Home will provide all medical care at home. If patients choose to visit you, Medicare Bulk Billing will not be feasible as they are still considered inpatients of Caulfield Hospital.

External Referrals:
We accept GP and specialist referrals to this service.
All referrals are triaged by the service according to clinical urgency. Patients requiring immediate assessment should be sent to the Emergency Department.
To refer you patient, complete and fax a referral to us. For urgent referrals, also contact the service Registrar to discuss the case.
To ensure appropriate and timely triage, include all demographics and clinical details as well as relevant investigation results.
If you are concerned about any delay of the commencement of the service or if there is any deterioration in your patient’s condition, please contact the service immediately on 0439 526 869.
Internal Referrals:
We accept referrals for any Alfred Health patients.
To refer a patient to the service, make an eReferral on PowerChart.
Under Orders & Referrals search for Better at Home and complete the required information.
Inpatients at The Alfred and Sandringham Hospitals will be reviewed by the Rehab and Aged Care Service (RACCS) team.
Inpatients at Caulfield Hospital will be reviewed by the Caulfield Bed Access unit as well as the Better at Home Clinical Coordinator.
