Clinical service overview
We focus on patient centred care from the treating team and involvement of family and significant others in the rehabilitation process.
The Rehabilitation Service cares for people who require rehabilitation in the following main areas:
- Amputee
- Orthopaedics
- Spinal
- Neurology
- Musculoskeletal
- Cardiorespiratory
- Burns
- Acquired Brain Injury
Each treating team is comprised of medical, nursing and allied health professionals such as physiotherapy, occupational therapy, social work, speech therapy, dietetics, prosthetics and orthotics, clinical psychology and neuropsychology.
Once people are discharged from the Rehabilitation Service, community services such as Bolton Clarke, Hospital in the Home, Therapy in the Home, Community Rehabilitation and Day Centre are integrated into the person’s care plan to ensure continued support, treatment and monitoring following discharge from hospital.
There are also a range of outpatient clinics to provide long term follow up following discharge from the rehabilitation service.

Call Caulfield Bed Access
We accept referrals for direct admission into this service from GP's and specialists. You need to call the Caulfield Bed Access Unit to arrange admission. You may also need to complete an Admission Request Form. Admissions are prioritised depending on patient needs and the patient and/or family is called regarding admission time and day.
