Post Acute Care
The Post Acute Care (PAC) program aims to facilitate a safe and timely discharge from hospital. The person returns home with an appropriate package of community-based supports. They are also linked with ongoing long-term support in the community if required.
Eligibility for Post Acute Care
People being discharged from a public hospital (including emergency departments, acute and sub-acute services) with limited supports at home can be referred to PAC. They need to be assessed as requiring short-term, community-based help to assist them to recuperate in the community, and to ensure safe and timely discharge.
Where another program was providing support prior to the hospital episode, it is expected that these services will resume on discharge from hospital.
Services offered by Post Acute Care
PAC support include a range of community-based services according to the person's individually assessed needs. The most common services provided are:
- Community nursing
- Personal care
- Home help
- Accompanied or unaccompanied shopping
Which Post Acute Care program should I contact?
PAC is a statewide Department of Health and Human Services program. The state is geographically divided up into different PAC services. Most patients who reside within the Alfred Health catchment are serviced by Inner South East Post Acute Care (ISEPAC), however all Alfred patients can access a PAC near their residence.
