The Aged Community Mental Health service (also known as MAPS) forms part of the Aged Psychiatry service located at Caulfield Hospital. We provide mental health services, including outreach assessment, treatment and support to older adults, mostly over the age of 65 years living in the Inner South of Melbourne.

Clinical service overview

We can provide:

  • Specialist psychiatric assessment, treatment and management for many mental health problems
  • Mental health case management to ensure complete and individual support
  • Care coordination with GPs, other specialists and community organisations that provide services to aged persons
  • Visits to clients in their place of residence, whether it is at home, an aged care home or supported residential service
  • Collaboration to achieve personal recovery goals
  • Support, information and referrals for family or carers and involvement in treatment decisions and planning
  • Sensitivity to cultural and linguistically diverse communities (including qualified interpreters and bilingual staff)
  • Education to clients, family, carers about living with mental health conditions
  • Education to aged care homes, aged care agencies and Non-Government Organisations on how to work with clients with mental health conditions
