The Aged Community Mental Health service (also known as MAPS) provides mental health services to older adults living in the Inner South of Melbourne. We are part of the Alfred Mental and Addiction Health service.

What we do

MAPS is part of the Aged Psychiatry service located at Caulfield Hospital. 

We provide the following services:

Support for clients:

  • Specialist psychiatric assessment, treatment and management for many mental health problems
  • Mental health case management to ensure complete and individual support
  • Care coordination with GPs, other specialists and community organisations that provide services to aged persons
  • Visits to clients in their place of residence, whether it is at home, an aged care home or supported residential service
  • Collaboration with patients to achieve personal recovery goals
  • Sensitivity to cultural and linguistically diverse (CALD) communities, including the provision of qualified interpreters and bilingual staff

Support for family and carers:

  • Support, information and referrals for family or carers and involvement in treatment decisions and planning
  • Education to clients, family, carers about living with mental health conditions

Community support:

  • Education to aged care homes, aged care agencies and Non-Government Organisations on how to work with clients with mental health conditions

We can offer help to persons over 65 years with mental health problems including (but not limited to):

  • Depression and anxiety
  • Schizophrenia and psychosis
  • Bipolar disorder
  • Psychological and behavioural problems that may be associated with dementia

What to expect

If you or a family member has been referred to our service, our team will phone you to make a time to talk to you and complete an assessment.

What to bring

Every time you come

  • Medicare card
  • Health Care Card and/or concession card (if you have one)
  • Private health insurance card (if applicable/if you want to use it)
  • Adverse drug alert card (if you have one)
  • Previous X-ray films, scans, ultrasounds or any other test results or reports
  • Medicines you need to take while you are here
  • List of medicines you are currently taking (or the boxes), including medicines you have bought without a prescription, such as herbal supplements and vitamins
  • Glasses, hearing aid(s) and/or walking frame if needed
