Community Diabetes Service
What we do
We provide education and support to people living with type 2 diabetes.
The diabetes education team includes:
- A diabetes nurse educator (DNE)
- A dietitian
- An exercise physiologist (EP)
The service helps you to:
- Improve your understanding of what type 2 diabetes is
- Learn to monitor your diabetes
- Learn what to do if blood glucose levels are higher or lower than you want
- Understand your medications
- Feel more enabled to make healthy lifestyle choices
- Learn how to reduce the risks of developing diabetes complications
- Feel more confident in managing your diabetes
- Improve your overall health and wellbeing
Who we care for
People over the age of 18 years with type 2 diabetes who:
- Are newly diagnosed
- Have long-term type 2 diabetes
- Need to start or have recently started any new diabetes medication including insulin or other injectable medication
- Want to learn more about self-management
Clients must live in the following areas to access this service:
- City of Glen Eira
- City of Stonnington
- City of Port Phillip
- City of Bayside (North of South Rd)
How to access this
Referral to Caulfield Access Unit
Referrals can be made by yourself, a GP, specialist, doctor or health care professional.
Referrals are triaged depending on priority.
