What we do
The Alfred's HRFS provides coordinated care for patients with predominantly diabetic foot complications, with a focus on preventing decline in vascular-compromised and high-risk diabetic foot patients.
Our aim is to provide personalised care plans and treatment inclusive of all health professionals involved in patient care.
Who we care for
People with diabetes often have an increased risk of foot ulcers due to impaired sensation and reduced blood supply. We provide care for people with foot wounds as a result of vascular disease and/ or diabetes.
What to expect
Our specialist team will work with you to complete a thorough assessment of your foot health. We will set goals with you to help improve your foot problem.
- You will receive treatment for your foot condition
- We will provide a letter to your doctor and other health care providers, to share your care plan
- The diabetes doctor and diabetes nurse educator can help you to manage your diabetes
- The vascular surgeon can see if you need further tests to help improve the blood flow to your feet
- We will give you a dressing plan for your wounds
- Our team will provide suggestions for footwear, and will provide special medical shoes to help with wound healing
How to access this
Referral from your GP or health practitioner
You need a referral letter from your GP or health practitioner to access this service.
Your doctor will need to fax your referral letter to us. We will be in contact with you in clinically recommended times, depending on waiting list length. If there is no waiting list, you will receive an appointment booking letter or we will contact you to arrange a suitable time.
What to bring
For a clinic appointment
- Your diabetes monitor (glucometer) or logbook
- Results of any blood tests and scans (if not perfomed at The Alfred)
- Your current orthotics or offloading devices
