What we do
We aim to work in partnership with patients and their families to improve independence, and/or reduce their long-term health needs or the need for community support. We provide a comprehensive assessment, treatment and management service.
Staff within the service work together as part of a interdisciplinary team and are highly skilled health professionals. As an inpatient, you will be able to access nursing, medical and support staff 24 hours a day. Allied Health support is generally available Monday through Friday.
Who we care for
We care for people with chronic or complex conditions associated with ageing, cognitive problems such as delirium or dementia, chronic illness or disability. We specialise in, but are not limited to, care for older people.
What to expect
We will work in partnership to achieve the goals identified by you and your family.
We will make every effort to get to know you, your individual preferences and usual routine.
We encourage you and your family to be as active and independent as possible during your stay.
Every day members of your treating team will meet with you. This is a great opportunity to ask questions and be involved in your care.

Rooms are generally shared with private rooms only available based on medical need.
Television and telephone services are available for hire.
Paid parking is available onsite.
How to access this service
Referral from your GP or specialist
Your GP or specialist can refer you for admission into this service by calling the Caulfield Bed Access Unit. The date of admission will depend on your care needs. The Bed Access Unit will call you or your family to arrange the day and time of admission.
What to bring
Every time you come
- Medicare card
- Health Care Card and/or concession card (if you have one)
- Private health insurance card (if applicable/if you want to use it)
- Adverse drug alert card (if you have one)
- Previous X-ray films, scans, ultrasounds or any other test results or reports
- Medicines you need to take while you are here
- List of medicines you are currently taking (or the boxes), including medicines you have bought without a prescription, such as herbal supplements and vitamins
- Glasses, hearing aid(s) and/or walking frame if needed
For overnight (or longer) rehabilitation stay
- Comfortable day clothes, such as a tracksuit
- Well-fitting, closed-toe styled shoes with firm grip soles. Do not wear open back, slip-on slippers
- Personal hygiene items, such as shampoo, shaving cream, shaver, toothpaste and deodorant
What not to bring for an overnight (or longer) stay
- Your handbag, wallet, purse or large amounts of cash
- Valuables, such as jewellery or watches
