The Diabetes Connecting Care Clinic (DCCC) is a service helping people with diabetes to access a broad and supportive clinical team. Our aim is to work closely with you and your usual GP / Doctor to achieve your diabetes goals.

What we do

Type 2 diabetes can be managed through lifestyle changes and medication. Managing diabetes well is the best way to prevent diabetes-related complications.

The supportive DCCC team will:

  • Provide you with clear guidance and instructions to help you to self-manage your diabetes
  • Help to improve your understanding of what type 2 diabetes is
  • Help to reduce the risks of developing diabetes complications and learn how to monitor your diabetes

Who we care for

This clinic is available to adults with type 2 diabetes and suboptimal glucose levels, requiring different specialists to clinicians to support their self-management.

What to expect

This clinic will be located at Caulfield Hospital and will involve staff from the Alfred and Caulfield Community Health Centre, known as your DCCC team. After a series of appointments over 6 to 12 months, and once you have achieved your goals, we will transfer care back to your usual GP through a virtual case conference.  If needed, your GP can always refer you back to the DCCC if you need more help with your diabetes.

How to access this clinic

Referral from your GP

You need a referral letter from your GP or medical practitioner to access this service.

Your doctor will need to fax your referral letter to us. We will be in contact with you in clinically recommended times, depending on waiting list length. If there is no waiting list, you will receive an appointment booking letter or we will contact you to arrange a suitable time.

What to bring

Every time you come

  • Medicare card
  • Health Care Card and/or concession card (if you have one)
  • Private health insurance card (if applicable/if you want to use it)
  • Adverse drug alert card (if you have one)
  • Previous X-ray films, scans, ultrasounds or any other test results or reports
  • Medicines you need to take while you are here
  • List of medicines you are currently taking (or the boxes), including medicines you have bought without a prescription, such as herbal supplements and vitamins
  • Glasses, hearing aid(s) and/or walking frame if needed

  • Your appointment letter
  • If you have been asked for blood tests, have these done within two weeks before you meet with the DCCC team
  • For each visit, bring along an up to date list of your medication, your glucose meter and blood glucose readings
  • Follow your care plan as agreed with your DCCC team

If you have diabetes

  • Your blood glucose meter
  • Your blood glucose diary/record book
  • Your favourite hypo food
  • Something to do, like a book, magazine or an iPad/tablet with headphones
  • Your diabetes medicines
